Click on “Start Service”.
Step Two
Fill the personal data.
Step Three
Upload the required documents (pdf, png, jpg, jpeg).
Step Four
Agree to the acknowledgment and complete the request.
Step Five
To follow up the request, an SMS will be sent to the registered phone number
